A lumbar medial branch block is a diagnostic and therapeutic injection used to pinpoint and alleviate pain originating from the small joints in the lower spine, known as the facet joints. This non-surgical procedure can offer significant clarity and relief for patients dealing with persistent discomfort. What Is a Lumbar Medial Branch Block?

A lumbar medial branch block is an injection of anesthetic near the medial branch nerves that supply the facet joints in your lower back. These nerves are responsible for transmitting pain signals from the facet joints to the brain.

If the block provides immediate pain relief, it confirms that the facet joints are the source of the discomfort. In some cases, the injection also offers temporary relief, making it both a diagnostic and therapeutic procedure.

Lumbar Medial Branch Block Benefits

This procedure is an effective tool in diagnosing and managing chronic back pain. Benefits include:

  • Provides rapid pain relief by targeting the specific nerves responsible for transmitting pain signals from the facet joints.
  • Allows for precise diagnostic insight, helping your provider determine whether the facet joints are the true source of your discomfort.
  • Reduces the need for oral medications, which can have side effects or limited long-term effectiveness for chronic pain.
  • Improves daily function and mobility, enabling many patients to return to normal activities more comfortably.
  • Offers a minimally invasive approach with no need for incisions or general anesthesia.
  • Acts as a gateway to longer-lasting treatments, such as radiofrequency ablation, if the block proves effective.

Who Is a Good Candidate for a Lumbar Medial Branch Block?

This procedure is typically recommended for patients who have ongoing lower back pain that may be related to facet joint inflammation or degeneration. If other diagnostic imaging or physical exams suggest that the facet joints are the source of pain, a medial branch block can help confirm the diagnosis.

Patients who experience stiffness, aching, or difficulty standing or bending—especially if these symptoms have not responded well to physical therapy or medication—may be excellent candidates.

Your Consultation

During your consultation at Core Alliance, Dr. Rovner will review your medical history, symptoms, and any prior imaging to determine if a medial branch block is appropriate for you. If it is, we will clearly outline what to expect and how the results of the procedure will guide your future care.

“Dr. Rovner was very quick to respond, courteous, answered all questions, and made sure I was able to comprehend the diagnosis. The practice managers were great as well. They were quick and efficient and helped me to understand the next steps. I would definitely recommend Core Alliance!”

What to Expect During Your Lumbar Medial Branch Block Procedure

The procedure is performed on an outpatient basis, usually taking less than 30 minutes. You will lie face down while the treatment area is cleaned and a local anesthetic is administered. Using fluoroscopy (real-time X-ray imaging), Dr. Rovner will guide a thin needle to the precise location of the medial branch nerves. Once in place, a small amount of anesthetic is injected near the targeted nerves. Patients typically remain awake during the procedure so that immediate feedback can be evaluated. You may be asked to perform simple movements to assess whether your pain has improved.

What Is the Recovery Like After a Lumbar Medial Branch Block?

Most patients can return home shortly after the procedure and resume normal activities within a few hours. You may experience some soreness at the injection site, but this is usually mild and temporary.

If the procedure successfully relieves pain, it confirms the facet joints as the source of discomfort and may justify proceeding with a longer-term treatment such as radiofrequency ablation.

What Is the Cost of a Lumbar Medial Branch Block?

The cost of a lumbar medial branch block varies depending on your insurance plan and the specifics of your treatment. Our team will provide a detailed cost estimate and discuss coverage during your consultation.

Alternatives to a Lumbar Medial Branch Block

If the results of a medial branch block are inconclusive or if the procedure does not provide the desired relief, other diagnostic and therapeutic options may be considered. These can include:

  • Radiofrequency ablation for longer-lasting nerve pain relief
  • Epidural steroid injections for nerve root inflammation
  • Physical therapy to improve strength and flexibility
  • Advanced imaging to reassess the source of pain

Frequently Asked Questions Lumbar Medial Branch Block

How long does pain relief from a medial branch block last?

Pain relief can vary. Some patients experience relief for several hours, while others may feel better for a few days. If relief is significant but temporary, you may be a candidate for longer-term solutions like radiofrequency ablation.

Is the procedure painful?

Most patients report only mild discomfort during the injection. A local anesthetic is used to minimize pain during the procedure.

How soon will I know if the procedure worked?

You may begin to feel relief within minutes of the injection. Your doctor will assess your pain level and physical function during and shortly after the procedure.

Can I drive after the procedure?

It is generally recommended to have someone drive you home, even though sedation is not typically used. This ensures safety while the anesthetic wears off.

Will I need additional procedures?

If the medial branch block is successful, your provider may recommend a follow-up treatment such as radiofrequency ablation to provide longer-lasting pain relief.
